From the long range camera it looks like Kobe planted and threw first. It was Horry yanking on Crosheres' jersey. The first "thing" to hit the ground was a ref.

Kobe's first punch looks like a little girl. Looks like skinny little Reg was takin it to him when they finally broke it up. watch Reggie knock Kobe back towards the scorers table.
