Do teams have an obligation to promote "Captains" who set the right example off the court as well as on it? Is this a slap in the face to the league and it's already not so hot image? Or does the team have the right to choose whoever they want as captain regardless?
I guess that team decided that he projects good enough mage and influence on his teammates and refs to call him a captain. Whether it is good or bad I don't know, but that's what they decided to do.

I don't think they are required. The qualifying offers are to maintain player in restricted status. If a player signs an offer sheet with another team, Cavs can match that.
So do they become unrestricted by not signing? If you don't need to sign the qualifying offer to remain a free agent, what's the point of extending it? Oh man, I hate the idea of trying to read parts of the CBA again.
